The problem

Most autonomy programs die between the pilot and the operation.

The vehicle usually works. What stops the program is everything around it: an operational domain nobody defined, a safety case that cannot be evidenced, stakeholders who were never brought in, and no one inside the organization who owns the thing once the vendor leaves.
